This calculator converts a dose you have already been prescribed. It does not recommend a dose. Never change your dose or schedule without talking to the clinician who manages your treatment.

How to use the TRT Dosage Calculator

The calculator has three tabs. Most people only need the first. The other two help when you change products or get blood results back.

    Tab 1: Dose to mL

    1. Pick your product and strength. Check the vial or ampoule. Most cypionate is 200 mg/mL. UK enanthate and Sustanon are 250 mg/mL.
    2. Enter the vial size so the tool can tell you how long it will last.
    3. Enter your prescribed dose and choose whether it is per week or per injection.
    4. Pick your injection schedule, for example once or twice a week.
    5. Pick your syringe. Choose the insulin syringe option if you inject with a syringe marked in units.
    6. Read the results. You get the mL and units per injection, the testosterone content per week, a schedule comparison table and any warnings.

    Tab 2: Switching products

    1. Choose what you use now and enter your weekly dose.
    2. Choose the product you are moving to.
    3. See the dose that gives the same amount of testosterone, and the mL it takes up.

    Tab 3: Blood test units

    1. Enter your total testosterone result and pick nmol/L or ng/dL.
    2. Tell the tool when the blood was taken in your injection cycle.
    3. You get the result in both units, where it sits on a reference range, and a note on timing.

Testosterone mg to mL: the formula

Every TRT dose calculation comes down to one line. Divide the dose by the strength on the vial.

mL per injection = mg per injection ÷ strength (mg/mL)
mg per injection = weekly mg ÷ injections per week
    U-100 syringe units = mL × 100

How many mL is 100 mg of testosterone cypionate?

At 200 mg/mL, 100 mg is 0.5 mL. On an insulin syringe, that is 50 units. At 250 mg/mL, the same 100 mg is 0.4 mL, or 40 units.

How many mL is 200 mg of testosterone?

    At 200 mg/mL, 200 mg is exactly 1 mL. At 250 mg/mL, it is 0.8 mL.

    A common slip

The strength on the vial is not the dose. A “200 mg/mL” vial does not mean you take 200 mg. It only tells you how much testosterone ester is in each mL of oil.

How much testosterone per week on TRT? What guidelines and labels say

There is no single “normal” TRT dose. Doses are adjusted to bring blood testosterone into a target range. These are the example regimens in official guidance and product labels.

    SourceProductExample regimenWeekly average
Endocrine Society (2018)Enanthate or cypionate75–100 mg weekly, or 150–200 mg every 2 weeks75–100 mg
US cypionate labelTestosterone cypionate50–400 mg every 2 to 4 weeks12.5–200 mg
UK Sustanon 250 SmPCSustanon 2501 mL (250 mg) every 3 weeksAbout 83 mg
UK Nebido SmPCUndecanoate 1000 mg/4 mL1 injection every 10 to 14 weeks. The first gap may be shorterAbout 71–100 mg

The Endocrine Society’s goal is a testosterone level in the mid-normal range, measured midway between injections. That is why two people on the same dose can end up on different doses a few months later.

TRT injection frequency: weekly vs twice weekly vs every other day

Splitting a dose does not change how much testosterone you take each week. 100 mg once a week and 50 mg twice a week are the same weekly total. What changes is the shape of your blood levels.

    • Less frequent injections give a higher peak after the injection and a lower trough before the next one.
    • More frequent injections give smaller peaks and troughs, so levels are steadier.
    • Evidence is limited. Steadier levels make sense on paper, but there is little trial data showing that people feel better on frequent schedules. It is a choice to make with your prescriber.

The table under the calculator shows what your own dose looks like on each schedule. More frequent schedules mean smaller volumes, which is one reason some clinics use insulin syringes and subcutaneous injections.

Testosterone enanthate vs cypionate: dose conversion

Injectable testosterone is attached to an ester, a chemical tail that slows its release. The ester adds weight. So 100 mg of “testosterone cypionate” is not 100 mg of testosterone. This is the part most TRT calculators skip.

EsterTestosterone per 100 mgRelease speedCommon products
PropionateAbout 84 mgFastPart of Sustanon 250
EnanthateAbout 72 mgMediumUK Testosterone Enantate, US generics
CypionateAbout 70 mgMediumDepo-Testosterone and US generics
    Sustanon 250 blendAbout 70 mg (176 mg per 250 mg)MixedSustanon 250
    UndecanoateAbout 63 mgVery slowNebido, Aveed

Figures for single esters are worked out from molecular weights. The Sustanon figure comes from its UK product information. In practice, cypionate and enanthate are so close that most prescribers treat them as one-for-one. The gap matters more when moving to or from undecanoate. Tab 2 of the calculator does this maths.

Sustanon 250 and Nebido dose calculations

What is in 1 mL of Sustanon 250?

Each 1 mL holds four esters: 30 mg propionate, 60 mg phenylpropionate, 60 mg isocaproate and 100 mg decanoate. Together that is 250 mg of esters, equal to 176 mg of testosterone. It contains arachis (peanut) oil, so it must not be used by people allergic to peanut or soya.

    How is Nebido dosed?

Nebido is 1000 mg of testosterone undecanoate in 4 mL. It is given as a deep injection into the buttock muscle, very slowly over about two minutes, every 10 to 14 weeks. It is given in a clinic, so you do not draw it up yourself. The calculator includes it so you can compare its weekly average with other products.

Testosterone levels: nmol/L to ng/dL conversion

Blood testosterone is reported in two units. The UK, Canada and Australia mostly use nmol/L. The US mostly uses ng/dL. The conversion is simple:

    ng/dL = nmol/L × 28.84
    nmol/L = ng/dL × 0.0347

A large 2017 study set a harmonised range of 264–916 ng/dL (about 9.2–31.8 nmol/L) for non-obese men aged 19 to 39. Labs may use different ranges, so always compare with the range on your own report.

When should blood be taken on TRT?

The Endocrine Society suggests measuring testosterone midway between injections for enanthate and cypionate. A test taken just before your next injection shows the lowest point. A test taken a day or two after shows the highest. Tab 3 reminds you of this so you do not read a peak or trough as your “normal” level.

Can shilajit raise testosterone or replace TRT?

This is one of the most common questions we get. The honest answer is no, shilajit cannot replace prescribed TRT.

    • What the research shows. One small 90-day trial in healthy men aged 45 to 55 found that 250 mg of purified shilajit twice a day raised total testosterone compared with placebo. That is a single small study in men who did not have diagnosed low testosterone.
    • What it does not show. No study shows shilajit can treat low testosterone or work like TRT. The rise in that trial was modest, and no one has tested shilajit in men who need TRT.
    • If you are on TRT. Tell your prescriber before adding any supplement that claims to affect hormones. It can make your blood results harder to read.

If you are curious about shilajit for general wellbeing, see our guide on how much shilajit to take per day.

    TRT safety: what to monitor

    • Haematocrit. Testosterone can thicken the blood. The Endocrine Society advises stopping treatment if haematocrit goes above 54%, until it is checked and managed.
    • Blood pressure. Product labels list raised blood pressure as a possible effect, so it should be checked regularly.
    • Prostate checks. Guidelines include prostate cancer risk assessment during the first year of treatment.
    • Fertility. TRT can reduce sperm production. Tell your prescriber if you plan to have children.
    • Oil reactions. Rarely, oily injections can cause cough or breathlessness straight after the injection. Get medical help if this happens.
    • Only use prescribed products. Testosterone bought without a prescription may be fake, contaminated or the wrong strength. If the strength is wrong, every number in this calculator is wrong too.

    Common TRT dosing mistakes

    1. Confusing the vial strength with the dose. 200 mg/mL is the strength, not how much to inject.
    2. Forgetting to halve the dose when splitting. Twice a week means half the weekly dose each time.
    3. Mixing up mL and units. 0.5 mL is 50 units on an insulin syringe, not 5.
    4. Switching products mg for mg without checking the strength and ester.
    5. Reading a peak or trough blood test as your average level.

    Frequently asked questions

How many mL is 100 mg of testosterone cypionate?

At the usual 200 mg/mL strength, 100 mg is 0.5 mL, or 50 units on a U-100 insulin syringe. At 250 mg/mL, it is 0.4 mL, or 40 units.

How many units is 100 mg of testosterone on an insulin syringe?

    On a U-100 insulin syringe, 1 unit is 0.01 mL. So 100 mg is 50 units at 200 mg/mL and 40 units at 250 mg/mL.

How much testosterone cypionate per week is typical on TRT?

The Endocrine Society’s example regimen for cypionate or enanthate is 75 to 100 mg a week, or 150 to 200 mg every two weeks. Your actual dose is adjusted by your clinician based on blood tests.

Is 200 mg of testosterone a week a lot?

It is double the upper end of the Endocrine Society’s example weekly regimen. It also equals the top of the US cypionate label range of 400 mg every two weeks. If you have been prescribed this much, it is worth asking your prescriber how your blood levels and haematocrit are being monitored.

Is testosterone enanthate the same dose as cypionate?

Very close. 100 mg of enanthate holds about 72 mg of testosterone, and 100 mg of cypionate holds about 70 mg. Most prescribers treat them as one-for-one, but check the mg/mL strength, because enanthate is often 250 mg/mL.

How do I convert testosterone nmol/L to ng/dL?

Multiply nmol/L by 28.84. For example, 18 nmol/L is about 519 ng/dL. To go the other way, multiply ng/dL by 0.0347.
